Your First Visit
On your first visit, we will take a thorough case history and perform an examination including your vitals, posture screening, general movement evaluation, orthopedic tests, and neurological exams. This exam will help us truly understand the problem that could be creating your symptoms. We will also discuss any physical, chemical (diet), or emotional stressors that could prevent you from feeling your best. At this point, we will provide you with a report of findings that include our findings, prognosis, treatment plan, or referral, if needed. If you choose to undergo treatment, it could include chiropractic manipulative therapy (adjustments), home care and exercises recommendations, and/or manual therapies.
In order to be most efficient, we ask that you arrive 10-15 minutes before your scheduled appointment time to fil out all necessary paperwork. If this is not feasible, new patient paperwork is available for downloading, printing, and filling out in advance. If you would like our office to file claims with your insurance, please bring your insurance card and photo ID to your first appointment. If you have had any recent imaging or you have imaging that you feel would be useful, please feel free to bring it with you.

While insurance can be confusing to figure out, we will do our best to determine what your insurance coverage is. We are currently in network providers for Blue Cross and Blue Shield. If this is not your insurance, we are currently accepting “cash” or self-pay patients! If you have insurance you wish to submit to on your own, we are more than happy to provide you with any documentation needed.
All fees for services will be discussed before treatment.
